The Physicians Circle for Patients (PCP) wants to help you!
PCP offers a top notch Patient Assistance Program (PAP). This program accepts and assists anyone who needs help affording their medications. Our trained Eligibility Specialists help patients navigate through the complicated world of insurance, pharmaceutical jargon and medical documentation to make sure each patient attains their medications. The program charges a $30 annual administration fee and a $10 service fee per approved 90 day prescription. There is no limit to the amount of PAP applications we will assist you with.

Do you need healthcare assistance?
PCP offers a free medical care program for qualified patients. The medical care program is ONLY for patients who are uninsured and fall below the 150% Federal Poverty Line. Qualifed patients pay nothing for any treatment. PCP is able to offer these services through the gracious donations of our member physicians and volunteers. Currently, to gain access to this program a patient must receive a recommendation from a member physician and complete the Georgia Volunteer Health Care Program form (below), as well as provide proof of income. We are currently unable to offer open enrollment for this program.
A patient may join either program or both. However, it is important to note that the free health care a PCP patient receives will be free of charge and that the only thing they are required to pay for is assistance with their prescriptions and this service is optional.

Any paperwork or Proof of Income may be delivered to the office via mail or in person between the hours of 7:00 AM and 7:00 PM. However, if you need to sign paperwork or speak with a PCP staff member, you will need to visit the office between 8:00 AM and 5:00 PM unless special arrangements have been made.

Picking up Medications
If you have been notified that your prescription is ready for pick-up, please stop by the office at your earliest convenience and receive your medication. Please remember that there will be a $10 charge when you pick up your medication. Your annual enrollment fee of $30 will also need to be paid before you can pick up any medications.
All payments must be made in cash or money orders. We cannot accept checks or credit/debit card transactions.
